READ CAREFULLY – THIS IS A WAIVER AND RELEASE OF LIABILITY

I desire to voluntarily enroll my child(ren) to participate in physical activities and/or a performance in conjunction with an event organized by OCSC Parties (the “Event”). I understand and expressly assume that there exists certain inherent dangers and risks of damage or injury associated with participating in the Event. My minor child(ren) have/has no existing medical condition and I have no knowledge of any condition in my and/or my minor child(ren)’s medical history that would prevent him/her/them from participating fully in physical activity in conjunction with the Event. I further certify that: (i) my child(ren) is/are not currently under the care of a physician for an injury or illness that would prevent his/her/their full participation in the Event; (ii) my child(ren) has/have no history of fainting or other medical diagnosis related to strenuous exercise; (iii) there is no reason he/she/they cannot safely participate in physical activities as expected by the Event without causing physical injury to himself/herself/themselves and/or others. I understand that the novel coronavirus diseases (SARS-CoV-2) and any resulting disease (together with any mutation, adaptation, or variation thereof, “COVID-19”) is extremely contagious and there is an inherent risk of exposure to COVID-19 in any place where people are present and exposure to COVID-19 can result in being subject to quarantine requirements, illness, disability, and other short-term and long-term health effects, including death, regardless of a person’s age or health condition.

In consideration for allowing my minor child(ren) to participate in the Event, I hereby agree to release, waive, covenant not to sue, and forever discharge Orlando Sports Holdings, LLC (“Orlando City SC”), Major League Soccer, L.L.C. (“MLS”), Soccer United Marketing, LLC (“SUM”), Orlando Pride, LLC (“Orlando Pride”), National Women’s Soccer League, LLC (“NWSL”), the owners of the fields and facilities utilized for the Event (“Property Owners”), and each of their respective officers, employees, agents, successors, parents, affiliates, subsidiaries, sponsors, advertising and merchandising agencies, licensees, and assigns (collectively, the “Releasees”) from all liability to my minor child(ren), our personal representatives, heirs, assigns and next of kin for any and all injury, damages, accidents, or death which may occur to my minor child(ren) during my child(ren)’s participation in the Event, including any liability which is attributable to the negligence of the Releasees, or otherwise. In the event of an accident, injury, sickness, or other condition needing medical attention, and until I can be contacted, I hereby give my consent and permission for any and all medical attention and treatment to be administered to my child(ren). I further agree that I am and shall be responsible for all costs associated with any injury or loss that may be sustained by my child(ren) as a result of his/her participation in the Event. I agree and acknowledge that in the event of a medical emergency, OCSC Parties will call emergency medical services and render assistance and that I will be financially responsible for any expenses related to such services.

Further, I hereby consent and grant permission to Orlando City SC, MLS, SUM, Orlando Pride, NWSL, Property Owners, and each of their respective officers, employees, agents, successors, parents, affiliates, subsidiaries, sponsors, advertising and merchandising agencies, licensees, and assigns (hereinafter the “OCSC Parties”) to use my child(ren)’s name(s), photograph(s), video(s), and/or other indicia of my minor child(ren), worldwide and in perpetuity, for any purpose, by any means and in any medium, whether now known or hereinafter developed. I understand that I shall not have any right to review or approve of any material within which my minor child(ren)’s photograph and/or video may appear, and I understand that the OCSC Parties may use such photographs or videos with or without associating names thereto. I further waive any claim for compensation of any kind for the OCSC Parties use and/or publication of photographs and/or videos of my minor child(ren). I hereby fully and forever discharge and release the OCSC Parties from any claims for damages of any kind (including, but not limited to, invasion of privacy; defamation; false light or misappropriation of name, likeness or image) arising out of the use or publication of photographs and/or video of my minor child(ren) by the OCSC Parties, and covenant and agree not to sue or otherwise initiate legal proceedings against the OCSC Parties for such use or publication on behalf of my minor child(ren).

All grants of permission and consent, and all covenants, agreements, and understandings contained herein are irrevocable. I represent and warrant that my minor child(ren)’s participation in the Event, including but not limited to, my child(ren)’s participation in any physical activity or performance, will not reflect badly on the OCSC Parties. I represent and warrant that only the minor child(ren) registered and listed hereto will participate in any activities in conjunction with the Event. I acknowledge and represent that this Waiver and Release of Liability is intended to be as broad and inclusive as permitted by the laws of Florida allow and if any portion of this Waiver and Release of Liability is invalid, the remainder shall continue in full legal force and effect. I acknowledge and represent that I am over the age of eighteen (18), that I have read this entire document, that I understand its terms and provisions, and that I have signed it knowingly and voluntarily on behalf my minor child(ren).
